Below are the steps you need to follow in order to report any cards recieved during a game:
After the game that a red or yellow card was given out the manager must report the cards when reporting the score, and the coach must fill out an incident report by following the steps below:
- Report the score and cards recieved as normal through the state system.
- After you report the score/cards, follow this link to get to the incident report form that needs to be completed to report the cards:
- http://www.bollingersoccer.com/site/ClaimsV1/?id=8:
- This form serves as a place for the coach to give his side of the story to the state, so that they are not just going off of what the referee turned in.
- This form is also used to report injuries, so be sure you only fill out the appropriate information.
- Also make sure that you click the incident button at the top of the form instead of the injury button.
- If there is a field in the form that is required, but not applicable, simply type N/A into the line.
- If you have any questions about filling out the report, please feel free to contact us, 770-641-3706.
